The Southwest Washington Wound Care Challenge: Finding the right location for your specific leg wound needs
Understanding Leg Wounds and Their Treatment
Leg wounds, especially VLUs, are often linked to chronic venous insufficiency. These ulcers can be slow to heal and frequently recur, significantly impacting patients’ quality of life. At Inovia Vein, we focus on comprehensive wound care that includes advanced treatments like endovenous ablation, which has shown promising results in recent studies.
Evidence Supporting Early Vein Ablation for VLU Care
Recent clinical trials provide strong evidence for the effectiveness of early endovenous ablation in treating VLUs. Two significant studies—the Early Venous Reflux Ablation (EVRA) trial and the VLU trial—have highlighted the benefits of early intervention.
- EVRA Trial: This randomized controlled trial demonstrated that patients receiving early endovenous ablation combined with compression therapy experienced a significantly shorter healing time compared to those who deferred treatment. The early ablation group also showed a higher healing rate at one year and a lower recurrence rate.
- VLU Trial: This study reinforced the findings of the EVRA trial by showing that early endovenous sclerotherapy led to faster healing rates and fewer recurrences compared to deferred intervention.
These and other trials collectively indicate that early intervention not only improves healing rates but also reduces long-term complications associated with VLUs.
National and international vascular guidelines emphasize the importance of seeing a specialist for venous leg ulcers (VLUs), recommending that patients undergo vascular imaging studies to accurately diagnose the underlying causes of their wounds. These guidelines also advocate for rapid access to both ablation procedures and comprehensive wound care, as timely intervention significantly enhances healing outcomes and reduces the risk of complications.
Leg Wound Care in Vancouver and Southwest Washington
At Inovia Vein, our approach to leg wound care includes:
- Thorough Initial Evaluation: We assess each patient’s condition comprehensively to develop a personalized treatment plan. The goal is to determine the cause of the ulcer as a venous leg ulcer, diabetic foot ulcer, arterial blockage or other.
- For VLUs, we provide in office specialty Vein Treatments shown to enhance how quickly leg wounds heal and how they stay healed such a venous ablation.
- Compression can be a useful adjunct, but it is important to understand the vascular status of the limb and the cause of the wound first
- Basic Wound Care and Dressing Changes: We utilize specialized dressings to create an optimal healing environment. This helps clean up the wounds so good cells can move in and close the wound. We find one of the more effective approaches for VLUs is the Unna’s boot, as it provides both a medicated dressing and compression and it can be worn for a week between visits.
- Debridement Procedures: Necessary debridement is performed to remove dead tissue and promote new growth.
- Biopsy if needed: Occasionally a non healing wound needs a biopsy to help understand why it is not healing.
- Advanced Skin Substitutes: For chronic wounds, we offer advanced skin substitutes, known as CAMPS, that have been shown to enhance healing.
All of this is covered by most insurance carriers and we can work with you to determine in advanced what is and what is not covered. It can be complicated, but we have a team working with you to understand your insurance benefits so you can make informed decisions about the care you wish to have.
