Following National Standards of Rapid Access to Modern Vein Care

Based on the EVRA trial, early referral for vein treatment is recommended for patients with Venous Leg Ulcers (VLU).  The EVRA trial revolutionized our approach to venous leg ulcers (VLUs). This landmark study compared early endovenous ablation to deferred treatment. Results showed faster healing and lower recurrence rates with early intervention. Consequently, patients should now receive specialist referrals within two weeks of ulcer onset. Moreover, early ablation significantly accelerates healing, reducing pain and improving quality of life. Additionally, it may prevent ulcer recurrence, potentially lowering long-term healthcare costs. Therefore, primary care physicians should promptly refer patients with VLUs to vascular experts. This is especially crucial for ulcers present for 6 weeks to 6 months with signs of superficial venous reflux. By implementing these findings, we can enhance outcomes for VLU patients. Ultimately, early intervention leads to faster healing, less recurrence, and improved patient satisfaction.

 Vascular Studies

After the initial evaluation, we perform comprehensive vascular imaging. We do this conveniently in our office. If you have a wound on your leg vascular studies are imperative to help determine if he cause is venous, diabetic or arterial or some other category.  This includes venous reflux and arterial studies to ensure adequate circulation. These tests help us identify the root cause of your leg wound and determine the best course of action. If we discover a diabetic foot ulcer (DFU) or critical limb ischemia (CLI) ulcer, we refer patients to appropriate specialists for further care.  If its venous in origin, which most are, we can care for you in our setting.

 Vein Treatments

Modern vein treatments have revolutionized the management of varicose veins, offering patients effective and minimally invasive options. There are two main categories of treatments available today. First, ablation techniques target the saphenous veins, the main superficial veins in the legs. These include radiofrequency ablation (RFA), VenaSeal, polidocanol endovenous microfoam (PEM), and mechanochemical ablation (MOCA). Each of these methods aims to close off problematic saphenous veins, redirecting blood flow to healthier vessels.

 

Of note, surgical vein stripping, once a common treatment for varicose veins, is now rarely performed. This traditional procedure has fallen out of favor due to its higher costs, increased complications, and lack of superior results compared to modern, less invasive approaches.

The second category focuses on branch tributary treatments, which address smaller varicose veins branching off from the saphenous veins. This includes sclerotherapy, where a solution is injected to collapse the vein, and phlebectomy, a minimally invasive surgical removal of surface veins. Modern minimally invasive techniques have largely replaced vein stripping, offering several advantages:

The application process is simple: we apply the CTP in our office and cover it with an Unna’s boot. For stubborn wounds, patients typically need 6 to 8 weekly applications.
